Under Assessment Instruments, viewers will find downloadable copies of the Treatment Services Review and several versions of the Addiction Severity Index, including Spanish and Native American versions. This page also provides a toll-free number where viewers may call to obtain more information on the ASI. Manuals providing supplementary material on these instruments are available for download. Programs and explanatory material for calculating ASI composite scores (researchers only) may be obtained from both these pages.
